An anal fissure is a tear or break in the skin of the anal canal. It is a small tear in the thin, moist tissue that lines the anus. Anal fissures generally occurs when a person passes hard/ tight/ large stools during a bowel movement & making him strain more which causes fissure in anal region (a small cut). Anal fissure can be noticed by bright red anal bleeding sticking to the stool or may be falling in little small drops during stool or in washing the anal region after stool.
Anal fissures could be very painful during and after defecation. Anal fissure usually extend from the anal opening & are usually located posteriorly in the midline, probably because of the relatively unsupported nature & poor perfusion of the anal wall in that location. Fissure depth may be superficial or sometimes down to the underlying sphincter muscle.
1- Most commonly the anal fissures are caused by stretching of the anal mucous membrane beyond its capability such as in case of tight/hard/large stools.
2- Superficial anal fissures look much like a paper cut & may be hard to detect upon visual inspection, generally they heals naturally on its own within few weeks.
3- Some anal fissure becomes chronic if left untreated and gets deep & will not heal on its own. The most common cause of non-healing is spasms of the internal anal sphincter muscle which results in impaired blood supply to the anal mucosa. Thus results in a non-healing ulcer, which may become infected by faecal bacteria.
4- Most common cause of fissures in adults is Constipation, the passing of large, hard stools, or by prolonged diarrhoea. Whereas, in older adults anal fissures may be caused by decreased blood flow to the area.
5- Some sexually transmitted infections can promote the breakdown of tissue resulting in a fissure. Sexually transmitted infections that may affect the anorectal area are syphilis, herpes, chlamydia & human papilloma virus.
6- Other common causes of anal fissure are:- Anal sex, ulcerative colitis, crohn’s disease, childbirth trauma in women.
1- Severe discomfort during bowel motions. A sharp severe pains when you pass stools, often followed by a deep burning pain that may last several hours.
2- After bowel movement discomfort that may continue for several hours.
3- After a bowel movement, bright red blood may be seen on stools or on a toilet paper. Sometimes a streak of line of blood is visible on stool representing a cut in anal mucosa.
4- A skin break/ tear is clearly apparent around the anus.
5- A skin tag (small lump of skin) next to the anal fissure.
1- Piles are mainly the swollen blood vessels while fissures are kind of cracks or tear in anal mucosa.
2- Piles may be painless or painful but fissures are never painless. Fissures are always painful and burning while passing stool and after stool also.
3- Both Fissure & piles are result of constipation and passing of hard stool
4- Piles is also caused by sitting for long hours, sedentary habits, constipation and putting strain or pressure on blood veins in the anal area whereas Fissures are caused by regular passing of hard stools or doing it often cuts the skin of the anal area.
